Making a referral

All CPS contract referrals require a 2054, a 5303 referral form, and an affidavit or service plan. CPS Case Workers can make referrals by faxing Avalon at (210)481-7175 or emailing avalonsa@sbcglobal.net

Note: CPS Case Workers can refer clients for counseling services on an Avalon Referral Form. Click Here This may be helpful in situations where the CPS case is being closed, but the Case Worker would like the family to continue with counseling sessions (for Medicaid clients).
